3. Not at one point did Kyoko or Madoka get through to her in the second fight, also indicating lack of intelligence.

CONCLUSION:
1.As Sakaya was a Mahou shojou to begin with, she was already exponentially powerful so as a witch, the same would apply.
2. As such, within a day she dispersed a familiar which grew up at an exponential rate into a witch.
3. The new witch, although different, retained some of Sayaka’s memories, but not all of them, this is shown by the fact that she is not acknowledging Madoka or Kyoko.

4. The real witch is either hiding, or preparing for Walpurgis night.

Walpurgis Night most likely powers witches, for a Mahou Shojou witch it can be assumed that MS witches seek to bind themselves with the event thus increasing their own power.

I notice that Madoka's mother was absent again in this episode. So what if:

-Madoka's mother is fired for having tried to take her boss's place. She kills herself. But as she was a Mahou shojo when she was young, she becomes a witch.
- Last boss is Madoka's mother. Madoka realizes that not only her mother is dead, but also Homura will have to fight and kill her.
- She makes her wish for all to 'reset' ('subete ga moto ni modoru'? That can be interpreted a LOT of different ways)
- We're back at the beginning of times. Homura who understood the way Madoka's wish would be (once again) interpreted, followed her through time.Madoka and Homura are alone in an empty universe. Like we can see in he ED, Madoka enters into a state of artificial sleep (... incubation? Her wish was granted, and Kyuubee is an incubator, something like that has to happen...)
-The magic in her grows AGAIN during all the time that is needed for Kyuubee to meet her in the future.
That's why her power is the one of a God and even Kyuubee doesn't understand why.
This is also the reason why Homura can't stand the idea of being left without her/loosing her, and also why Madoka is the center of 'her' universe: she has been alone watching over her, for way too long.

At least Mami's moments, we got to see some badass moves from her.

An otherwise healthy young girl suddenly dropping dead usually prompts an autopsy. In any case, a reasonable attempt to find what caused her demise would undoubtedly be made.

Personally, it'd be more interesting to me if there were evidence or eyewitness accounts of Sayaka killing those guys on the train (supposing that they're dead). She doesn't need to be conclusively proven to be their murderers; the mystery and possibility just needs to be there. She'll go down as one of those urban legends that middle school girls gossip about during sleepovers. It'd be ironic--the wanna-be whiteknight-in-the-shadows ultimately goes down as a questionable villain in the public's eye.

Madoka will die.

But her soul will chose to go back in time, to the time when she was happiest, and she will be back at the first episode.

Homura, who loves Madoka wants to be with her too and in a time when Madoka loved her, but since she already loves Sayaka, she'll go back in time before Madoka is born, and became her mother.
