It's assumed, but never discussed, that there is a larger world outside of Equestria and that this too would be fucked, but the show makes a conscious effort not to think about it and doesn't talk about the world outside of Equestria. Children, by extension, are probably not going to think about the world being threatened or saved, just Equestria. Conceptualizing the world in the show would require demonstrating things like what the planet looks like, who lives there, etc. and for characters to start talking about how these places would also be affected by the world-threatening plots of the pony nation. If Nightmare Moon returned and threatened night eternal to a group of ponies, Griffons, and Dragons who together exclaimed the end of the world was near, they'd be demonstrating "saving the world". The pilot was just saving Equestria, while saving the rest of the world was incidental and not thought of.
